Terms & Conditions

Effective: October 10, 2026

About these Terms

These Terms apply when you visit the Rewilding Integrative Health, PLLC (“the Practice,” “we,” “our”) website, use our scheduling or patient portal, buy our resources, or become a patient. Using any of these means you accept the Terms & Conditions. Treatment is additionally covered by the informed consent forms you complete with your clinician. For anything clinical, those forms and the law take priority over this page.

The Practice

Rewilding Integrative Health, PLLC is organized in Minnesota and offers virtual integrative and functional health / medicine care. Clinical services are delivered by Kimberly McNally DNP CNM FNP-C FMP-C, who holds a Minnesota license.

Who can become a patient

  • You must be at least 18.

  • You must live in Minnesota and be located there when you attend appointments, since our clinician’s license is state-specific.

  • The information you give us must be truthful and complete. You must tell us promptly when your medications, health status, contact information, or billing details change.

How virtual care works

Appointments are held by video or phone from a private place of your choosing.

Telehealth has trade-offs. It saves travel, but it doesn’t allow a hands-on exam, and a dropped connection can interrupt a visit. If that happens, we’ll try to reconnect or set a new time. If your clinician decides your concerns need in-person attention, they’ll tell you so and direct you as applicable.

You are free to see a local provider instead, and you can revoke your agreement to telehealth whenever you like. We’ll then talk through next steps, including handing your care off to someone else.

Emergencies: This service cannot respond to urgent situations. Dial 911 or go to the closest emergency department. For a mental health crisis, call or text 988.

Patient status: You become our patient only once you’ve had an initial consultation and we’ve agreed to create a patient-provider relationship following consultation. Visiting the site or purchasing a digital resource does not create a patient-provider relationship.

Clinical care

  • Your clinician determines clinically-appropriate options in coordination with you after personal evaluation. Scheduling a visit or submitting intake forms does not entitle access to any specific test, treatment, or prescription.

  • Your plan may draw on lab or specialty testing [such as blood, urine, hair tissue mineral analysis, stool, hormone, or nutrient panels], nutrition and lifestyle coaching, supplements, and prescriptions where warranted.

  • Outside laboratories perform most testing. [Certain tests are not FDA-cleared and are rarely covered by insurance; your clinician will explain how to interpret the results and their limits.]

  • Supplements are not meant to diagnose, treat, cure, or prevent disease, and the FDA does not assess them for those uses. It is in client best interest to disclose everything taken in order to properly determine interactions and outcome possibilities. It is not possible to accurately predict all outcome scenarios.

  • Prescriptions are issued to a pharmacy and are for your use only. Passing them to someone else is a violation of the law. [Compounded medications have not been approved by the FDA, and the agency does not evaluate their safety, efficacy, or quality. We’ll talk with you about whether compounded options are clinically appropriate for you.]

  • We reserve the right to refuse care or stop prescribing if continuing would be unsafe or clinically inappropriate.

  • Maintain regular relationship with your primary care clinician. We support that relationship and aren’t a replacement for it.

What we ask of patients

Be candid about your health history and everything you take, follow the plan you and your clinician agree on, tell us promptly about side effects or worries, finish requested labs and follow-ups, and send clinical questions through the secure portal. Please be courteous. We will discontinue care in response to threatening or abusive behavior.

Payment and cancellation

  • Prices for visits appear when you book. [Membership or program charges, along with any lab or supplement costs, are disclosed before you’re billed.]

  • [Recurring charges continue until you cancel or pause, which you can do by contacting us.]

  • We operate on a cash-pay basis and do not submit insurance claims. A superbill is not available, though FSA / HSA card may be used for payment.

  • Cancellations and refunds follow our Billing and Refund Policy, which forms part of these Terms.
